RELEASE OF LIABILITY

I, the undersigned, accept full responsibility for my health and voluntarily complete this release and waiver of liability form. 

I certify that I am seeking the services of May Levy of Deep Intuitive Healing for:

1. Intuitive Healing - Intuitive Healing is a form of alternative healing that focuses on releasing emotional blockages in the body, combining pressure point massage, balancing the energy channels (chakras) within the body using sound therapy like overtones and light language. Deep Intuitive Healing offers treatments for people who are looking for an alternative approach to healing their mind, body, and soul and may help to heal from issues such as: ancestral baggage stored in the DNA,  karmic patterns, trauma in this lifetime and past lifetimes, chronic body pain, injuries, and even disease.

2. Theta Healing - According to Theta Alchemy, our brains have 5 basic frequencies, that is, Gamma, Beta, Alpha, Theta, and Delta, and we usually use them one at a time; however, one frequency tends to be dominant, depending on the situation. In the phase of deep meditation, sleep, or hypnosis, the dominant wave is the Theta wave, and scientists have concluded that this frequency has the ability to lower stress and anxiety, lead to deep relaxation, enhance mental clarity and creativity, minimize ache, and increase euphoria. During Theta Healing, the brain enters this wave within minutes, and the individual works with the Creator, God, or the Universe (depending on one’s beliefs) to bring about healing.

3. Channeling -channeling information is an ancient practice and it’s a way to amplify the whispers of healing the past or stepping into your destiny. Channeling sessions can help clarify a problem, allow insight into a disorder, or spark creativity when it comes to moving forward into the world and getting specific about what you are attempting to cultivate and also what may be holding you back. Receiving and acting on deep personal truths emerging out of the limits of this space/time paradigm can be immensely empowering and transformational.

I acknowledge and agree that these services are considered to be for educational purposes only. I fully understand that the services provided by Deep Intuitive Healing are not designed or intended to diagnose or cure a medical condition or to serve as a substitute for diagnoses or treatments determined by a licensed medical doctor, chiropractor, physical therapist, and/or any other licensed specialist, and I represent and warrant that I am not seeking the services for medical diagnostic or treatment purposes.

I certify that with respect to any medical conditions or concerns I may have, I have been advised to consult with my licensed primary care physician.  I also understand that it is my responsibility to discuss any and all information provided during Deep Intuitive Healing consultations and coaching with my primary health care provider or any other health care providers/specialists whose care I may be under.  All information received by me from Deep Intuitive Healing is accepted with full knowledge that any action or inaction taken by me as a result of the information received is my complete responsibility. 

I certify that all information provided by me to May Levy and Deep Intuitive Healing with respect to my physical, mental and nutritional health is complete, truthful and accurate.

I, on behalf of myself and my successors, heirs, and assigns, hereby release and hold harmless May Levy and Deep Intuitive Healing, and their respective representatives, agents, employees, successors, heirs and assigns, from and against any and all liabilities, losses (including death, personal injury and illness), damages, costs, expenses and any other claim relating to or arising from my involvement with May Levy and Deep Intuitive Healing.

I understand that these services are NOT COVERED by INSURANCE and that I am responsible for all costs incurred.

I understand that May Levy is NOT a licensed medical physician.

I understand that the treatment or guidance provided by Deep Intuitive Healing is alternative or complementary to healing arts services licensed by the state and that these products or services provided are not licensed by the state.

I acknowledge that, while some results can be experienced immediately, I may also benefit from multiple sessions. Each person’s healing is unique, and each healing environment is approached in that way, so my session(s) with May Levy and Deep Intuitive Healing will target my personal needs.  Techniques and methods are 100% intuitive so variations to treatments are naturally applied and variations in results naturally occur.

CANCELLATION POLICY 

Should you need to cancel or reschedule, please notify me and receive confirmation of cancellation via text, email or booking system 48 hours in advance. Any cancellations made between 48 to 24 hours notice are subject to a cancellation fee of 50% of the session rate. Any cancellations made with less than 24 hours notice or any sessions that are missed with no notice, are subject to a cancellation fee equal to the fee of the missed session.

By signing below, I am acknowledging and agreeing that I understand all the terms set forth in this release and waiver of liability and consent to be bound by the same.
